How do we learn from our mistakes (and the things we did right?) The best thing to do is to conduct a postmortem meeting.

It was an interesting list of things to do, but I think a much more useful article is "how to avoid a witch-hunt." Post-mortems are usually only done when Really Bad Things happen, and there is a need for heads to roll. I think a "how not to" list would be much more useful than a "how to" list.
